Client intake form

Prior to you first session, please complete this form. This is very important.

Session duration and fees

Your first session is 80 minutes in duration, and costs $200 + GST.

Your follow-up sessions are 50 minutes in duration, and cost $150 + GST.

Payment

All sessions that are booked online are paid in advance at the time of booking.

Bookings made in person can be paid in advance or at the time of the session.

Cash and credit cards are accepted.

Online Hypnotherapy Sessions

All online sessions are held via Zoom. You do not need a Zoom account however it is helpful to have the app installed before your session.

Your Zoom meeting link will be emailed to you directly.

Minimum number of sessions

There is no contractual requirement for a minimum number of sessions however it is strongly recommended that you plan for at least three sessions to achieve a result.

Rescheduling or cancelling your session

You may reschedule or cancel your appointment via the booking session until 1 week before your appointment. 

Whilst I accept cancellations and rescheduling up to 72 hours before an appointment, you will need to contact me directly if your appointment is in less than a week.

Late cancellation and non-attendance

Late cancellation fees are payable as follows: 0-24 hours’ notice – full session fee payable. 24-48 hours’ notice – 50% of session fee is payable. 48-72 hours’ notice – 25% of session fee is payable.

If you do not attend a session, the full fee is payable.

Session records

For record-keeping purposes, every session is recorded. Face-to-face sessions are recorded as audio. Zoom sessions are recorded as audio and video.

The recordings and associated paperwork are stored securely and remain confidential.
